Your Future Job, In Summary In close collaboration with the coordinating nurse and/or care team, you will intervene at the patient's home to: Apply medical prescriptions and perform technical, relational, and preventive acts to support daily life and help maintain autonomy. Follow prescriptions and care protocols established by the prescribing doctor and the coordinating nurse. Gather information that may influence the patient's health status. Update and maintain the patient's medical records. Participate in developing and reevaluating the individualized care plan.
